The forms below include all of the forms a homeowner might need to apply for and take advantage of the available New York State heating and cooling rebate and incentive programs from NYSERDA.

If you are using a paper copy to fill out a document, please remember to scan it as a PDF before sending it back. However, if you are filling out an application digitally there is no need to scan it as a PDF. Once you have completed the application, please send the filled-out form to energystarwise@gmail.com.
